IMPORTANT BACK TO SCHOOL DAYS INFORMATION!

THIS SECTION IS FOR PARENTS/STUDENTS WHO HAVE RECEIVED THE SUMMER MAILER AND WERE ENROLLED BY THE END OF JUNE.
 
The following forms must be completed, printed, and brought to the Back to School Days in August:
 
9th GRADE PACKET   
__ PYLUSD Community Service Permission
__ Dance Agreeement
__ Military Opt-Out Form
__ Southcoast Pictures (If purchasing pictures)                   
 
10TH, 11TH, 12TH GRADE PACKET
__ Dance Agreeement
__ Military Opt-Out Form
__ Southcoast Pictures (If purchasing pictures)
 
OPTIONAL
PSAT Payment Information (this test is ideal for 10th and 11th)
PTSA Parent Volunteer Opportunities
 
IMPORTANT NOTE!!!!!!!
In addition, you must bring all forms that were mailed to your home address (including the forms from the PYLUSD Parent Handbook). If you did not receive a summer mailer, you won’t be able to complete the Back to School Days process.
